We would like to remind all prospective students that our enrollment specialists are available to answer any questions you may have before completing the application. Our enrollment specialists will be able to give you an overview of our various course offerings, what to expect while enrolled at QDA, and information on how to complete the application packet.
Please ensure that you meet the following criteria before you complete our application.
Students must be between the ages of 5 and 22. Students over 22 can enroll in our Adult 22+ High School Diploma Program. More information can be found at: adult.go2qda.org
Students must be legal residents of Ohio. Proof of Residency is required before we can enroll any student.
It is the responsibility of the student or their parent/guardian to obtain Internet access before starting their coursework. QDA does provide Internet reimbursement up to $40/month. QDA offers student the option to use one of our computer systems while they are enrolled in our program. Students can also use their own person computer or mobile device if they wish. You will be provided a computer system once you complete the orientation process.
Please complete the application below to start the enrollment process. Please note, once we receive your application we will request records from the last school you attended. Record requests can take one to five business days depending on the school district we are requesting records from. We can't process your application until we receive these records. We will contact you if we are experiencing delays in obtaining your records.

After you submit the application the QDA enrollment department will begin the enrollment process. A QDA staff member will contact you when we have all the required information to schedule an orientation or if we need additional information. QDA also requests that you complete two additional forms for Title-I and E-Rate purposes. Those forms can be accessed here.

QDA will require proof of Ohio residency and custody papers (if applicable) before we can complete the enrollment process.
QDA will contact you to schedule an orientation once we have all the required documents.
You will be officially enrolled AFTER you complete the orientation process. DO NOT WITHDRAWAL from your current school until you complete the orientation process. If you withdrawal before you are enrolled at QDA your current school may file truancy charges against you.
