Student Application

Prospective Students may download our application here. You may either complete the application on your computer system or print it off and complete it by hand.

You may also complete an application packet request and we will mail you all the required documents and a pre-stamped return label.

• Emergency Medical Card*
• US Department of Education Ethnicity Form*
