1) Please complete all the required fields that have the red flag to the right of the input box.
2) You can either draw your own signature or select the type-to-sign option from the drop down. When prompted for a signature the parent, legal guardian, or students over the age of 18 must sign the document.
3) When you have filled in all the required fields, the Submit Signature box will no longer be grayed out. Press it to submit the application.
4) After you click submit, you should be redirected to a page that has your application with a blue ribbon on the left hand corner.

Please remember: You are not enrolled in QDA until the student orientation has been completed. Do not withdraw from your current school.
