Who Can Enroll?

Any student under 22 who is a legal resident of Ohio.

Student Application

The application can be downloaded and completed entirely on your computer system. After you have completed the application, please save it to your desktop. This will ensure that you can easily locate the application when you upload it using our application submission form.

  • Digital Signatures
    Our electronic application requires the use of digital signatures instead of a traditional signature. After you electronically sign the first field, you will be prompted to save the document. Please note, after the last signature the application will be locked and you will not be able to edit the form.

Proof of Ohio Residency

Using the Online Application Submission Form
 
The online submission form is the fastest way for us to receive your application. Whenever possible, please use this form to submit your application. 
  1. Complete the Application and Save it to your desktop. Please include your last name in the file name so we can easily locate it for future use.
  2. Go to this site and submit your application along with any additional documentation you can provide in electronic form.
Acceptable Format
  • PDF - The application will be saved in PDF format if you complete it on your computer system.
  • JPG/JPEG/PNG - If you scan additional documents to your computer, you may submit them if they are in this format.
  • ZIP - You may place all of the required documents into a single folder and zip it before uploading it. Please note, our server only accepts documents under 20mb in size.
